Regarding the Mishna quoted in this post:
It is like marriage and divorce - there are lot of "issues":-).
One issue is that the Mishna begins by stating כל גט שנכתב שלא לשום אשה פסול, כיצד telling us from the "get-go" that the invalidation we are about to discuss is a lack of לשמה for the woman [i.e. that it wasn't written for her sake] and then it goes to tell us two cases where the problem is a lack of לשמה AND for HIS SAKE לשמו!!? This is SOMETHING!!! הלא דבר הוא:-)!
[Reminds me of the perek Tehillim that begins מזמור שיר ליום השבת and then doesn't mention Shabbos for the rest of the chapter! הלא דבר הוא!]
Moreover the source for לשמה is the pasuk וכתב לה and we darshen לשמה. But what is the source for לשמו??
A third issue: There is a machlokes rishonim whether we require שליחות of the husband to write the get. According to those who hold that we do require שליחות, shouldn't THAT be the פסול in the Mishna [in addition to a lack of לשמה].
