1. On a scale of 1 to 10 how would you rate yourself as regards being

generally calm?

2. In what environments do you feel calm?

3. In what environments are you not calm?

4. Around which people do you feel calm?

5. What thoughts help you feel calm?

6. What thoughts prevent you from feeling calm?

7. When you are not calm, in what significant ways are you different

from when you are calm?
