Something About Our Yetzer

Every day in davening we say [where?] the following sentence:

יְ-ה-וָ-ה אֱ-לֹהֵי אַבְרָהָם יִצְחָק וְיִשְׂרָאֵל אֲבֹתֵינוּ שֳׁמְרָה זֹּאת לְעוֹלָם לְיֵצֶר מַחְשְׁבוֹת לְבַב עַמֶּךָ וְהָכֵן לְבָבָם אֵלֶיךָ

What does it mean?? What should be our kavana as we say it? Where does the sentence come from??
